Sentara Home Care is currently hiring a full time Care Transition Coordinator.

The Care Transition Coordinator is responsible for the identification and assessment of home care needs of patient population and coordination of these services to ensure a smooth transition to the next site of care.

The Care Transition Coordinator will be based out of Sentara Albemarle Medical Center in Elizabeth City, NC.

Shift: Monday - Friday (8am-5pm)

Requirements:

  • 1 year of Home Care/Hospice, Discharge Planning OR Case Management experience required.
  • Licensed RN, LPN, RT or BSW required.
